Hospice is not a place. It is a way of caring. When a cure is no longer possible, Hospice allows the care to continue. Hospice care is a way to provide genuine and meaningful support to a person with a terminal illness – allowing for the most comfort. I will assist you and your family with Hospice Care in your home or in a residential care community, wherever your loved one resides. I will guide you to an understanding of what your loved one is experiencing and what to expect as they transition through the end of life. In addition, I can:
- Work closely with family, church clergy and hospice to plan for end-of-life care
- Plan and arrange funeral services
RESEARCH RESIDENCE OPTIONS
- Work with families to determine if a loved one can remain in their home with in-home support services.
- If higher level of care is needed, and moving to an outside care facility is the best and safest option, will research multiple facilities, their services and associated costs to help families make informed decisions. Help make moving arrangements.
MANAGE PERSONAL NEEDS
- Manage client appointments and arrange personal transportation, airline reservations; can travel with client to out-of-state family visits.
- Oversee care at Assisted Living or Skilled Nursing Facility.
- Regularly visit clients and advocate for their needs.
ONGOING DIALOG
- Understand that health conditions and situations can change; remain open to meeting regularly to discuss new circumstances and needs.
COMMUNICATE WITH MEDICAL, LEGAL & FINANCIAL PROFESSIONALS
- Work closely with physicians and medical personnel in addressing client’s specific medical history and current needs.
- Work with estate planners, fiduciaries, attorneys, church clergy, realtors and other professionals to ensure the client’s needs are professionally managed, always with their best interest in mind.
- Serve as client’s primary caregiver if a family member or Power of Attorney lives too far or is unable to address timely needs or emergency medical situations.
- Be appointed Health Care Agent on Advanced Health Care Directive to make medical decisions and be direct point of contact between doctors and family. Many doctors appreciate the clarity of a single channel of communication.
